Welcome!  By accepting the terms and information below, you can contact our office electronically 24 hours a day 365 days a year.  This line of communication is best suited for requesting refills, scheduling appointments or requesting a call back for simple or non-urgent matters and questions.  Please note:

  • Most emails will be responded to within 24 hours, but please allow up to 72 hours for a response, especially during holidays and weekends.
  • The service is only to be used for Non-Urgent, Non-Emergent communication.
  • If you have an emergency or life threating concern, please contact emergency services immediately by dialing 911 from your telephone.
  • If you have an urgent matter please contact our office or the on-call healthcare provider at 360-495-3500.
  • Your email will not be responded to via email. Instead, you will be contacted at the phone number you provide to insure privacy, a prompt response and clear communication on our end.
